Improved Communication with Healthcare Providers: By completing the questionnaire and sharing the results with your healthcare provider, you can improve communication and help your provider understand your health concerns and history more fully.

Personalized Health Recommendations: Based on your responses, the questionnaire can provide personalized recommendations for improving your health and preventing future health issues. This may include guidance on diet, exercise, stress management, and other lifestyle factors that can impact your health.

Greater Control over Your Health: By understanding your health risks and taking preventive action, you can take greater control over your health and potentially reduce the risk of developing more serious health issues in the future.

Overall, the Holistik Health Review questionnaire can serve as a valuable tool for staying proactive about your health and taking steps to prevent future health issues.

  • What are the kinds of questions asked to determine HHR?

The Holistik Health Review questionnaire includes questions related to various aspects of a person’s health. Some of the kinds of questions that may be asked to determine the HHR include:

Medical History: Questions about any past medical conditions or surgeries, current medications, and any ongoing medical treatments.

Physical Health: Questions about current physical symptoms or health concerns, such as pain, fatigue, or difficulty with daily activities.

Family Health History: Questions about any family history of medical conditions, such as heart disease, diabetes, or cancer.

Lifestyle Factors: Questions about diet, exercise, sleep habits, stress levels, and other lifestyle factors that can impact overall health.

Mental Health: Questions about any symptoms of depression, anxiety, or other mental health conditions.

Women’s Health: Questions related to women’s health, such as menstrual cycle regularity, menopause symptoms, and sexual health.

Substance Use: Questions related to alcohol, tobacco, and drug use.

These questions can help assess a person’s current health status and identify potential areas of concern that may require attention or intervention to promote optimal health and well-being.

  • Is the Holistik Health Review universally accepted?

The acceptance and recognition of the Holistik Health Review questionnaire would depend on the policies and standards of individual hospitals, healthcare providers, insurance companies, and regulatory bodies.

While the questionnaire may be a useful tool for assessing an individual’s health status and risk factors, it does not replace the importance of seeing a licensed healthcare professional for all healthcare related needs.

  • vision

  • Who is eligible to take an online vision test?

In general, to be eligible for Visibly’s online vision test, you must

    • Reside in a U.S. state that allows an online vision test (view a list of active states).
    • Be between the ages of 18-55 (view more information about the age details).
    • Have no health conditions or take any medications that may impact your vision.
  • When should I use the online vision test?

Our test helps you renew your glasses or contacts prescription from home by evaluating your visual acuity in your current correction and sending the results for review by a licensed doctor in your state.

It’s best suited for people whose vision has not changed, have recently completed a comprehensive eye exam, and are:

    • Looking to renew an expired prescription, or
    • Looking to purchase glasses or contacts

If our test detects that your vision has changed, we will not renew your prescription and recommend seeing an in-person doctor.

The Visibly Digital Acuity Product does not perform or replace an initial eye examination. Consult an eye care professional if you still need an eye examination.

The Visibly Digital Acuity Product does not perform or replace a comprehensive eye examination or assess eye health. Consult an eye care professional for a comprehensive eye examination yearly or any time you are experiencing pain or discomfort. This test is not intended to diagnose, treat, mitigate, or cure disease.

  • Do I need a previous prescription to take the online vision test?

Yes. Visibly’s online vision test is a prescription renewal service, so the eye doctor will want to know your previous prescription so they know what they need to renew for you.

  • Do I need to be wearing my glasses or contact lens during the Visibly online vision test?

Yes. In order to receive a new glasses/contact lens prescription from us, you must be wearing your current glasses/contact lens. If you wish to renew an older prescription, we recommend you visit a local eye care provider to be refitted and approved with a new glasses/contact lens prescription.

  • Can I take the online vision test if I only have a prescription for one eye?

Unfortunately, we cannot accept patients who only wear a prescription for one eye at this time. Visibly’s system algorithm requires information to be entered for both eyes.

  • Can I use the Visibly online vision test to renew my driver’s license?

Currently, you cannot use the online vision test to renew your driver’s license. It requires that you have seen a doctor in-person who has determined your eyes are healthy enough to drive.
